Patient Registration

writingIf you wish to register with the Practice, please call in at Reception and pick up a registration form, (GMS1).  This form needs to be completed and returned to Reception.  You will need to complete one form for each person requesting to register.  You will need to bring proof of your address with you to register as you must live in the Practice area to be eligible.

Although you will be registered with the practice as a whole, our Doctors prefer you to have a usual Doctor in order to get to know you better and to aid continuity. If you have a particular preference though, please let the Receptionist know when you book your appointments.

Once your registration is complete the local Primary Care Trust (NHS Mid Essex) will be notified and they will arrange for your medical records to be sent to us from your old surgery.  Please note that this can sometimes take several weeks.

We ask all new patients to attend a ‘New Patient Health Check’ with one of our Healthcare Assistants. This is to obtain your medical history whilst we await your notes from your previous practice.
